Abstract:

Background: neoplasms especially the malignant types are important health problems in all communities. gastrointestinal malignancies are amongst the leading causes of malignancy-related mortality and morbidity. This study was performed to determine one-year prevalence of gastrointestinal neoplasms by type, location of involvement and demographic factors. Materials and methods: In this existing data study medical records of all patients receiving the diagnosis of a gastrointestinal neoplasm in public and private hospitals in East Azerbaijan province, Iran were evaluated from Mehr 1378 to Mehr 1379. Results: 1822 cases of gastrointestinal neoplasms were recorded. The mean±s.e. of the subject’s age was 60.2±13.4 years and 63% of them were male. The highest frequency of malignant neoplasms was found in the 7th decade. Gastric, esophageal and colorectal malignancies comprised 36%, 23% and 16% of the total gastrointestinal malignant tumors, respectively. The most common type of gastric malignancies was adenocarcinoma (94%) while the most common esophageal malignancy was found to be squamous cell carcinoma (82%). Conclusion: Considering the remarkable increase in the prevalence of gastrointestinal neoplasms in the province during 5 years, investigations on the etiological factors and especially the food content and dietary habits of people seems necessary. Such investigations can be facilitated and fortified by more meticulous registration of cancers and developing specialized information and data registry centers for cancer patients.

Abstract:

BACKGROUND & OBJECTIVE: Mesenchymal tumor of the gastrointestinal tract comprises a diverse group of neoplasms which occurs in adults. With recognition of immunohistochemical profile in recent years, gastrointestinal stromal tumor (GIST) has been grouped separately from other sarcomas. No therapeutic modality except. Surgey has ever demonstrated definitive evidence of clinical benefit in GIST. These tumors occur most commonly in the stomach, followed by small intestine, and are rare in the colon and rectum and extremely rare in the esophagus. GIST also arises rarely in the omentum, mesentery and peritoneum.CASE: A 69-year-old male was admitted to surgical ward with clinical and CT scan findings of large, movable abdominal mass behind the stomach with probable origin from the pancreas. Explorative laparatomy revealed an 18 x 16 cm mass in lesser omentum. Pathological and immunohistochemical techniques revealed malignant GIST.CONCLUSION: gastrointestinal stromal tumor should be considered in the differential diagnosis of the abdominal mass.

Background: This study was designed to investigate the various extracts of Allium giganteum in term of their cytotoxic effects on gastric and colon cancer cell lines and then fractionation of extract with the most desired effects on cancer cells in comparison to the normal cell line. Materials and Methods: The A. giganteum air-dried bulbs were extracted in four solvents including aqueous, butanol, chloroform: methanol (9: 1), and dichloromethan extracts. Evaluating the cytotoxicity of extracts were done by MTT assay against HT-29, KATO-III, and HUVEC cell lines. The extract with The most cytotoxicity on cancer cells and lower toxicity on normal cell line was subjected to the fractionation by MPLC using a RP-18 silica gel column and in the next stage and the cytotoxicity of resulted fractions were determined by MTT assay. Results: Chloroform: methanol extract was the safest extract on HUVEC cells with great cytotoxic effects against cancer cell lines (IC50 17. 3 and 23 µg/mL, for KATO-III and HT-29, respectively). The results of MTT assay for 7 fractions of the chloroform: methanol extract showed that 5th fraction (E) was the most cytotoxic fraction (IC50 9 and 11. 2 µg/mL for HT-29 and KATO-III, respectively). TLC and NMR analysis of the fractions suggested the presence of steroidal saponins as the main constituents. Conclusion: Significant cytotoxic effects of Allium giganteum was established against GI cancer cell lines especially fractions rich in steroidal saponins. So, these fractions are attractive candidate for the isolation and identification of effective ingredients.

Abstract:

Introduction: Esophageal cancer (EC) is a common cancer of the digestive system which is one of the most common cancers in our country The primary treatment of EC is surgery. Due to the development of minimally invasive techniques (MIE), in the current study, we have assessed the results of these techniques in patients with EC surgery. Methods: A total of 80 patients with middle and lower third ECs who had good conditions and were operated with MIE technique (McKeown) from 2014 to 2021, were enrolled in this study. Patients were evaluated based on the following criteria: age, sex, tumor location, pathology, peri-operative complications leading the minimally invasive esophagectomy technique to being converted to an open surgery, and early post-operative complications after surgery and mortality. Results: A total of 80 patients with EC were enrolled in the study. 85% (n=68) of our patients were male and 15% (n=12) were female with an average age of 58. 21±, 11. 39 years old. 43. 75% of the patients had a history of neo-adjuvant chemotherapy. Surgery was performed with McKeown technique without complications in 91. 25% of the patients. In 8. 75% of the patients tracheal injury (n=1), uncontrolled bleeding (n=1), and severe pleural adhesions (n=5) led the surgery plan changing into open surgery. Post-operative complications were observed in 13. 75% of patients. Conclusions: This study suggests using McKeown technique in patients with EC in highly experienced medical centers in order to obtain proper results with low rate of peri-and post-operative complications.

Abstract:

Introduction & Objective: 40% lymphomas are extranodal and gastrointestinal (GI) tract is the most common site. gastrointestinal lymphomas by definition have not liver, spleen, lymph node and bone marrow involvement. The most common gastrointestinal lymphomas are MALTOMA, diffuse large B cell lymphoma (DLBL), burkitt, follicular and T cell lymphoma. The most common patients are old men. The aim of this study was determination of immunophenotypes of gastrointestinal lymphoma and relationship between the site, age and immunophenotype with two years survival.Materials & Methods: In this cross sectional descriptive study 53 primary gastrointestinal lymphoma were studied in admitted patients of Hamadan hospitals. Age, sex, and tumor immuno phenotypes (using with CD3, CD5, CD20, CD10, CD15, bcl2, lk light chains) and two years survival were recorded. Results were analyzed using SPSS 16 and chi-square test. Results: The most common age was 50-60 years and mild male preferential was seen.60% were MALTOMA and 24.5% were diffuse large B cell lymphoma.47.2% were in stomach and 30.2% were in small bowel. Total two years survival was 64.2% and in MALTOMA was 81.2% and in DLBL was 38.2% (P=0.005). In relationship between patient age and tumor site with two years survival there was no mean able relationship (P. value in first was 0.938 and in the latter was 0.308).Conclusion: The most common lymphoma was MALTOMA and DLBL. Two years survival in MALTOMA was better but age and tumor site have not relationship with two years survival.
